Should you wish to enrol your child you will need to know the following enrolment procedures.
Enrolment Details
You will be asked to complete an enrolment form containing information regarding your child’s health, development, custody arrangements and emergency contacts. You will need to name all persons who may deliver or collect your child from the Centre. These persons must be over 18 years of age. It is your responsibility to ensure your nominated people are responsible and available when required.
The details required on the enrolment form are needed by our staff to help them take the best possible care of your child. It is also a Licensing requirement All information is strictly confidential. If any of the details on the enrolment form change you are asked to advise the Director immediately.
At the time of enrolment you will be asked to fully disclose any medical or health concerns relating to your child, and agree to lodge your application with the Family Assistance Office for Child Care Benefit within 28 days of your child starting care.
